For incident reporting, please complete the form below. These reports will be immediately forwarded to the Designated Safety Officials.
You may use this form to report anonymously if needed. However, we do encourage you to include identifying information if possible for two reasons: one is that we'd like to be able to ask you further questions during our investigation process. We value your input in this process and we are dedicated to protecting you and making Shadow a safe environment. The second reason is that you deserve to know the outcome of the process. We'd like to keep you informed about how we handle your report.
